Family Income Sources

Add a separate response for each source of income.

This question seeks information relative to income earned through employment and self-employment.

You do not need to disclose the amount of income received.

You must disclose all instances of employment or self-employment during the relevant year in which you, your spouse or dependent child received $1,000 or more in gross income.  This also includes instances of public employment or service, including service in the public position you hold that necessitated this filing, where money (including a stipend) was received.  

First, answer whether it is income that you (the filer) earned, or whether it is income from a family member (your spouse or dependent child).  If income from a family member, you must enter the full name of the family member.

Next, answer questions regarding that particular income. Answer whether the income was from self-employment. 

Regardless of whether the income was from employment or self-employment, provide the name of the employer or business that was the source of income.  Then give your title or occupation and the dates of your employment or self-employment.  Provide the address of the employer or business that provided your income.

The last question applies only if your income was received through self-employment.  If so, answer whether you rendered services to a state or municipal agency for an amount of income in excess of $250.  This includes services rendered to regional authorities and quasi-public agencies.  If yes, list the date and nature of the services rendered to the state or municipal agency.
